How to Prepare for your first visit
Gather your pet's records
Have your pet's medical history sent to us. You can either have your previous clinic fax records,
or call us and let us handle the process for you. If you are bringing a new puppy or kitten, you can
either bring any records the breeder gave you or e-mail them to us at firstname.lastname@example.org.

Fill out our new client form
Want to be seen quicker? Fill out the new client form below to reduce your wait.
Bring the form with you to your appointment and hand it to our reception team at check in.
Bring anything else you may want to talk about at your visit.
Especially if your pet is nervous, many owners have found that bringing their pet's favorite treat helps in new and scary environments. If you are coming for a new puppy visit or an annual exam, you may also bring a stool sample to make the experience less stressful for your pet.
